Founding Circle
Founding rates are fixed for two years, then members receive 10% off the current standard rate.
Routine visits are outside insurance and do not require additional copays nor insurance billing.
Individual

Two Adults + Children

With the child add-ons at their maximum, it's approximately $7,700 / year billed annually or semiannually.
Pay annually or semiannually at the stated rate. Quarterly billing adds a nominal administrative fee. Monthly equivalents are for budgeting only.
Membership is annual, including installments. Cancel in writing anytime; refunds depend on unused whole months and services provided. The Patient Agreement governs.
Keep health insurance for specialists, hospitals, emergency care, imaging, medications, and outside labs. Membership complements your coverage; it does not replace it.
Medicare and TRICARE beneficiaries are not currently eligible for membership. If you have questions about your coverage, please contact us before submitting an inquiry.
